What to Expect from Our Animal Hospital
When you visit Boulevard Veterinary Hospital you will see excellent service and the best possible care for your pet from our veterinarians and professional staff. Efficiency is one of our main goals and that is why we try to get to know you here in the New Patient Center before you arrive.
If you have any medical history for your pet, please bring it when you come for your first visit. Some of our clients keep a journal of their pet's health throughout his or her life. This is not required, of course, but it is a good practice to get into because with information like this, we can serve your pet as best as possible. If you don't have information like this, don't worry. Just let us know everything that you find important. Our staff and veterinarians will work with whatever information you have.
Our animal hospital offers all of our patient forms online so that you can take your time and complete them comfortably and thoroughly. You can download and print out the forms and fax or email them back to us. You may also bring the completed forms with you to your appointment if you prefer.
Like most animal hospitals, payments are expected at the time of the visit. We accept many payment options such as debit cards and credit cards (Visa, American Express, MasterCard, and Discover) as well as checks and cash. Care credit and pet insurance is also accepted at our animal hospital.
